Referral for Special Education Evaluation

If you believe your child may have a disability, you may refer your child for an evaluation by submitting a request in writing. This must be a letter (handwritten or typed) with an original signature of the parent/guardian. Emails are not accepted. Please include the following important information in your referral letter: 

• Your name
• Your child's name
• Your child's school and grade level (if applicable)
• Your phone number and address
• Reason for referral or concerns

Submit your written request, with an original signature to the address below. You can mail your request or hand deliver it to Door 7 at Middlesex High School, Monday-Friday between 8:00 am- 3:00 pm.
